MASTER IN-SERVICE PROGRAM

CSF EDUCATORS

These forms are designed to expedite the Master In-service Program for your school.

There are two forms for each in-service activity. One copy of the IN-SERVICE

APPLICATION FORM is completed by the school’s In-service Coordinator for each In-service Activity. Each completed activity requires a CERTIFICATE OF COMPLETION and an EVALUATION SUMMARY. The forms are sent to the CSF In-service Office for approval and signature. This information is entered into the CSF database and returned to the school. Each school will be provided with a master of the forms necessary, which are also contained in the CSF Master In-service notebook.
