GUIDELINES FOR IN-SERVICE ACTIVITY

APPLICATION PROCEDURE

To be used whenever an in-service activity is to be initiated.

Submit the form to the CSF In-service Education Office at the end of the in-service activity. Should there be any question concerning whether the activity fits within the parameters of the Master In-service Program, please contact the CSF V.P. of In-service for verification of the program before the in-service date. The CSF In-service Office reserves the right to disapprove any project not fitting the MIP program. Advance permission is not necessary if the school is conducting an in-service activity already outlined in the Master In-service Plan.

PRIOR APPROVAL FOR INDIVIDUAL IN-SERVICE

(Application for In-service Points)

To be submitted by an individual who wishes to participate in in-service activities outside the home school, such as local public school in-service programs, professional conferences, professional state and national conventions. This in-service request can be approved by the local school administrator who can refer to the CSF In-service Education Office for advice concerning appropriate classification of the In-service activity. All outside in-service activities must remain within the MIP In-service program parameters in order to receive in-service points. The CSF In-service Education Office reserves the right to refuse in-service points to programs which in its opinion do not meet MIP in-service goals.When selecting a component from the MIP list, choose a component number that will enable you to earn points in your area(s) of certification. Points must be earned during the certification validity period of the participant. After in-service points are awarded, two copies of the Certificate of Completion are given to the participant. The participant must return one copy to the school to be placed in the personnel file; the other copy becomes the personal property of the participant.

WORKSHOP OUTLINE

An outline of the workshop must be submitted with the Application for In-service points. This outline, prepared by the workshop instructor, must identify the general objectives, specific objectives, activities and the evaluation process to be used in the workshop. The outline should be specific to the activity requesting approval. The outline may be used, as well in developing a new in-service component.

SESSION ATTENDANCE RECORD

The session attendance roll will include the participants full name, social security number and school. Participants must initial the roll each day. This roll becomes the permanent record of attendance and is filed in the CSF In-service office.

IN-SERVICE EVALUATION FORM

This should be duplicated at each member school and distributed, one per participant, at the end of the in-service activity. Each form must be completed and signed by the participant. These forms should then be summarized by the In-service facilitator on the In-Service Evaluation Summary, and the summary should be returned to the CSF In-service V.P. with the Session Attendance Roll. This summary evaluation will be made available to the State and maintained in the CSF In-service Office.

IN-SERVICE INDEPENDENT ACTIVITY – INDIVIDUAL RECORD SYSTEM

The in-service independent activity form is used to record independent activity that may be required to complete any in-service activity or individual professional growth activity. It must be signed by the instructor and participant.

CERTIFICATE OF COMPLETION

When a participant has completed the entire component of an approved in-service activity, Certificates of Completion are to be filled out by the participants with the assistance from the in-service coordinator. The instructor or coordinator must sign the certificate indicating that each participant has successfully completed the objectives of the component. One certificate of completion for each activity must be submitted for the signature of the CSF In-service V.P. Individual participants are responsible for and must maintain their own personal in-service record.
